QualityMetric Incorporated is the sole owner of the information
collected on this site. We do not sell, share, or rent this information to others in ways different from what
is disclosed in this statement. We do not accept any advertising on our web site.
QualityMetric Incorporated collects other information from you or your computer at several different points
throughout our website.
Demo (demonstration) Survey
A user may choose to complete a demo survey. No information from demo surveys is collected, saved or stored.
Order
We request information from the user on our order form. Here a user must provide contact information
(like name, phone number, shipping address, email address) and financial information (like credit card number,
expiration date). This information is used for billing purposes and to fill customers' orders. If we have trouble
processing an order, this contact information is used to get in touch with the user.
Data Scoring
We provide a service through our marketplace to score and or evaluate the quality of researcher's data.
We do not copy or save this data on our web site or anywhere else. We do not copy, save or share
this data with anyone. We return the data to the requestor immediately after the scoring has been completed.
Cookies
A cookie is a piece of data stored by a web site on the consumer's hard drive, which contains information about how the

When you request a single or multiple users non-commercial license for one of our products,
we ask you to complete a "Permission to Use/ Project Registration Form". A user provides

information to contact you if we have questions about your request or need additional
information to process your request. We ask detailed information about your project to
verify that you have chosen the correct license type and most appropriate survey for your
project. We ask you additional questions about the purpose of your project, number of
surveys to be administered, intervention being measured, publication intent, time frame,
data collection, measurement type and outcomes. This information is used to verify the
type of license requested, verify that there is no commercial intent and to determine a
royalty fee schedule (if any). We do not sell or share any of this requested information.
SF36® Partners Company Profile
When you request a commercial license we ask you to complete the "Partners Company Profile". A user must
provide contact information (like name, phone number, shipping address, email address). We use this
information to contact you if we have questions about your request or need additional information to
process your request. We ask questions about your business, like number of employees, annual revenues,
and product information. We request this information to create your contract, calculate your annual fee
and determine your royalty fee schedule. We do not sell or share any of this information.

We do not share any personally identifiable information, either individual or aggregate data, from this web site.
We use a third party credit card processing company, Verisign, to bill users for goods and services. This company
does not retain, share, store or use personally identifiable information for any secondary purposes.
Security
We take every precaution to protect the information you have shared with us. When users submit sensitive
information via our website, your information is protected both online and off-line. When our registration
form asks you to enter sensitive information (such as date of birth), that information is encrypted and is
protected with the best encryption software in the industry - SSL. While on a secure page, the lock icon
on the bottom of Web browsers such as Netscape Navigator and Microsoft Internet Explorer becomes locked,
as opposed to un-locked, or open, when you are just 'surfing'. While we use SSL encryption to protect
